Note: Resume should include any work experience, extracurricular activities, leadership positions, other honors and scholarship received.
Note: Essay should describe any accomplishments you have achieved, your strengths, your professional objectives / goals, your interests, why you selected your field of study, and why you should be selected to receive this scholarship.
Note: Transcript should be a current university (or high school if you are a senior) transcript.
Note: A minimum of one (1) personalized recommendation letter from a professional (employer, teacher, professor). The letter must be written and dated in the current year.

  • Verify all information provided is true and correct to the best of my knowledge.
  • Understand I may be contacted by telephone for a pre-selection interview at the discretion of the Member Chapter Scholarship Chair.
  • Upon knowledge of any changes to the information provided, will submit updated information.
  • Understand the decisions of the Member Chapters are final.
  • Understand I must start or be continuing in the course of study in the Fall of the same year the scholarship is awarded.
