Don B. Daily Safety Grant

Grant Acceptance

Your Don B. Daily Safety Grant proposal has been approved in the amount of US$20,000.

Note: A check will be issued payable to the university and mailed to the address above, unless noted otherwise.  
 
A steel industry mentor must be confirmed to assist you in carrying out your proposal and to act as a liaison to the Don B. Daily Memorial Fund Committee.  Grant recipients are encouraged to present the final results of this proposal at an SMA or AIST event.

A summary report must be submitted by 31 May.

I hereby accept the Don B. Daily Safety Grant and release the above information to the SMA and the AIST Foundation for the purpose of fund distribution.  I also authorize the use of my name and the university name for promotional purposes affiliated with the SMA and AIST Foundation.  In accordance with the terms of the grant , I agree to respond promptly to requests by SMA and the AIST Foundation for additional information including a summary report.
 
I understand that the grant is for one-year.  I verify that the information provided on this form is accurate and up-to-date and agree to notify SMA and the AIST Foundation immediately if any of the information I have provided changes after the submittal of this form.
