I hereby accept the Kent D. Peaslee Junior Faculty Award and release the above information to the AIST Foundation for the purpose of fund distribution.  I also authorize the use of my name and the university name for promotional purposes affiliated with the award.  In accordance with the terms of the grant l, I agree to respond promptly to requests by the AIST Foundation for additional information including a summary report.
 
I understand that the grant is for one-year and that re-application is required for subsequent years.  I verify that the information provided on this form is accurate and up-to-date and agree to notify the AIST Foundation immediately if any of the information I have provided changes after the submittal of this form.
